How to add a General Practitioner practice to a patient file

There are two ways to add a General Practitioner's (GP) details to a patient's record.

Note: To link a GP with a patient, you must have a role assigned to you that gives you editing permissions for Patients and Contacts. For more information on roles, see here.


Adding the GP directly from within the patient file by using the integrated GP directory

Edit a patient file and scroll down to "Relationships and emergency contacts" then click "Add relationship" as circled below:

A window will open on the right hand-side as below:

In the "Relationship type" field select "GP" as selected below:

Then start typing the first couple of letters of the GP practice you are looking for and select your choice from the drop down menu:

Add any details you may wish to add then click "Add relationship":

Don't forget to click "Save" in the top right hand corner of the patient file when you are finished:

You will now be able to see this GP relationships on the patient file as below:

Create the GP as a Contact

You must first create the GP as an external contact, from the Contacts page:

Click New.

Create the external contact as you normally would.

Make sure you have made the contact a Medical Practitioner, by ticking the Yes option here:

Click Save at the top of the screen.

The GP is now listed as a contact in your practice. You can now link the GP to a patient.


Link the GP to a Patient

When you have created the GP as a contact, you must then link them to the patient. You can do this from the Patients page:

Select the patient who you want to link the GP to.

Click the edit icon in the top right-hand corner.

Scroll down to the Relationships and emergency contacts section.

Click Add relationship.

For "Relationships type" Select GP from the dropdown menu as below:

Type the name of the GP that you want to link to the patient, you will first see choices from the integrated GP directory, but since you want to use a practice contact in this case, simply scroll down till you see "Check Practice contacts for matches":

You will then see the below section appear called "Practice contacts", click the one you would like to add to this patient file and then click "Add relationship":

Don't forget to click "Save" on the patient file once you are done.

The GP is now linked to the patient.
